The Notice for requesting the contribution for people with very serious disabilities is published.

FOR THOSE WHO DO NOT HAVE A CERTIFICATE OF VERY SERIOUS DISABILITY: In order to access the regional contribution, the interested person, a family member or guardian/support administrator must submit a request no later than 6 June 2025 to the competent Provincial Health Commission of Ancona located in Via C. Colombo n. 106, ast.ancona@emarche.it for recognition of the condition of very serious disability pursuant to art. 3 c. 2 of the Ministerial Decree of 26/09/2016. The request must be drawn up on the Attachment-B form (if the request is signed by the disabled person) or on the Attachment-B1 form (if the request is signed on behalf of the disabled person as a family member, guardian, support administrator, ...) . The request must be accompanied by: 1. report of recognition of the attendance allowance pursuant to Law 11 February 1980, n. 18 or pursuant to Attachment 3 of the Prime Ministerial Decree n. 159 of 2013; 2. specialist medical certification from a public or private facility that has an agreement and/or is accredited, drawn up according to the model in Annex C 3. valid identification document of the person with a disability 4. valid identification document of the person submitting the application as a family member or guardian or support administrator.

By 07 July 2027, the Health Commission will send the applicant, the Municipality of residence and the Marche Region, the certification relating to the recognition of the condition of very serious disability according to the Attachment-D Model . This certification is essential for the application for the contribution.

The disabled person or the parent or family member or the person exercising parental responsibility or guardianship or administration of support shall immediately and in any case no later than 16 July forward the request for contribution to the Municipality of Ancona, using the Form Attachment-E

FOR THOSE WHO ALREADY HAVE A CERTIFICATION ATTENTING THE CONDITION OF VERY SERIOUS DISABILITY It is not necessary to make a further request to the Health Commission: the disabled person or the parent or family member or the person exercising parental authority or guardianship or administration of support shall immediately and in any case no later than 16 July 2025 forward the request for contribution to the Municipality of Ancona, using the Form Attached-E

Applications for contributions must be sent to the Municipality of Ancona with all the attachments listed and required by the call NO LATER THAN 16 July 2025 using the following methods: - by certified email: comune.ancona@emarche.it - by registered mail with return receipt addressed to the Municipality of Ancona – Largo xxiv maggio, 1 – 60123 Ancona (the postmark is NOT valid) - by hand to the Social Services Protocol Office of the Municipality of Ancona Disability Unit V.le della Vittoria 37 (First Floor)

The application sent by registered mail must be placed in an envelope. On the front of the envelope, the following wording must be written: DGR M n.264/2025 – REGIONAL FUND FOR NON-SELF-SUFFICIENT PEOPLE 2024 YEAR - INTERVENTIONS IN FAVOUR OF PEOPLE WITH VERY SERIOUS DISABILITIES

Please note: Individuals who are guests in residential facilities are excluded from the contribution. The contribution is incompatible with other financial contributions provided for the same purpose of supporting home care, and is therefore ALTERNATIVE to the following interventions: - Care allowance” and “Independent Living” - “Independent Living” project pursuant to LR 21/2018, - Recognition and valorization of the care work of the family caregiver who assists people affected by Amyotrophic Lateral Sclerosis (ALS) - Recognition and valorization of the care work of the family caregiver who assists a family member aged between 0 and 25, affected by a rare disease pursuant to Annex 7 of the Prime Ministerial Decree of 12 January 2017 and who requires ventilation and/or artificial nutrition
